Eligibility

Eligibility Requirements

  • Aged 55 years old or older.

  • Live in the county of Alcona, Alpena, Presque Isle, or Montmorency or 48737, 48745, 48750 in Iosco.

  • Certified by the state as meeting the need for the nursing home level of care.

  • Able to live safely in the community with the help of PACE Northeast Michigan at the time you enroll.

  • While there are no financial eligibility requirements, PACE Northeast Michigan is covered by Medicare and Medicaid for eligible individuals.

  • PACE Northeast Michigan is also available on a private pay basis.

​What you pay for PACE depends on your eligibility under Medicare and Medicaid; If you are eligible for Medicaid, or dually eligible for both Medicare and Medicaid, you won't pay a monthly premium for PACE; If you don't qualify for Medicaid, but you have Medicare, you'll be charged a monthly premium for PACE; There's no deductible or copayment for any drug, service, or care approved by your health care team; If you don't have Medicare or Medicaid, you can pay for PACE privately.

There are three steps to enrolling into PACE Northeast Michigan

Intake Assessment

An initial Intake Assessment is completed to determine if a senior might be eligible for PACE Northeast Michigan. At this assessment, the PACE Northeast Michigan model of care is explained to the prospective participant.

PACE Northeast Michigan

Enrollment Assessment

At this point, the prospective participant will be scheduled for an assessment with the rest of the care team. Once the assessment by the PACE Northeast Michigan team is completed, a plan of care will be created for the participant.

PACE Northeast Michigan

Enrollment + Family Conference

A PACE Northeast Michigan team member will meet with the prospective participant and caregivers to talk about enrolling in the program. At this conference, the plan of care and enrollment agreement will be discussed and explained.
